You may qualify even if you have been discharged from physical therapy --- we bridge the gap between rehab and long-term wellness
How it Works
- Free Phone Intake: We learn about your goal and challenges
- Initial visit: Includes a movement screening and home safety check
- Weekly sessions: 1-2 visits per week with a personalized fitness plan
- Ongoing support: Optional family/caregiver training included
